When emergency personnel arrived, they encountered a vehicle on its top, partially underwater in Morgan Creek. The driver was trapped inside.

An investigation determined the driver had been traveling west on Highway 30 during heavy rain when he lost control and rolled in the median several times. The vehicle then rolled over the guardrail before coming to rest on its top, with the driver under water. The driver had to be "mechanically extricated" before being taken to Mercy Hospital where he was pronounced dead. He has been identified as 62-year-old Kevin H. Slater of Fairfax.

The investigation into the accident continues through the Linn County Sheriff's Office.
